DOT Physical

We welcome you to our quick, driver-friendly,

WALK-IN DOT clinic!

As the spouse of a professional, veteran, truck driver, our Practitioner understands the importance of compliance with the Department of Transportation and Federal Motor Carrier Safety Administration (FMCSA). We strive to keep the public and our professional drivers safe, and on the road, to provide for their families and maintain livelihood.

Please Bring

  • Active/unexpired Driver’s License

  • List of prescription and over-the-counter medications

– Include medication doses on the list or bring medication bottles

– Include name, address, telephone number of prescribing doctors

– Due to numerous medication side effects, use of ANY prescription medications will require a signed safety medical opinion letter from the prescribing provider stating the medications are well-tolerated and the side effects do not interfere with you driving a commercial motor vehicle (CMV)

REMINDERS TO PREPARE FOR THE APPOINTMENT

Each driver is unique. Therefore, these guidelines are not all-inclusive or a guarantee of passing the exam. Additional testing may be required to obtain a DOT card. Failure to bring the required items and documentation will cause a delay in certification.

Sky

If you have any of the following health conditions, please comply

Vision

– Bring your reading and/or prescription glasses, and contacts

– NOTE: If you are not able to pass the Vision part of the exam: Download the Vision Evaluation Report

– The medical examiner must receive the Vision report, not more than 45 calendar days, after an ophthalmologist or optometrist signs it

Hearing

– Bring your hearing aids

– If the whisper test is not passed, further testing is required with an Audiologist (hearing specialist) before you can be certified

Heart

(i.e.: stents, valve replacement, pacemaker, open-heart surgery, cardiac bypass surgery, heart attack)

– Bring recent (within the past year) Stress Test, Echocardiogram, or other heart tests (results must show Ejection Fraction %)

– Bring a letter from your Cardiologist that outlines your medical history, current medications, and that indicates it is safe for you to drive a CMV

Hypertension

– Your BP must be below 140/90 on the day of your exam to receive maximum certification of 1 year

– Take your medication as prescribed

– Maintain a low-salt diet

– Do not smoke or drink caffeine prior to your exam

Smokers

– Spirometry testing must be performed on all smokers over the age of 35
– Bring your Spirometry test results to your DOT exam
– Do not smoke or drink caffeine prior to your exam

Sleep

– Bring recent (within the past year) sleep study, multiple sleep latency test, or maintenance of wakefulness test results

– Bring CPAP usage report (previous 90-day usage, showing at least 70% compliance)

Diabetic (Non-insulin)

– Bring recent (within the past 3 months) A1C lab results

– Bring your electronic blood glucose self-monitoring records for the preceding 3 months

Diabetic (Insulin-dependent)

Download the Insulin-Treated Diabetes Form

– Have your treating physician fill the form out and bring it with you to your DOT exam

– The medical examiner must receive this report, not more than 45 calendar days, after your treating provider signs this report

– Bring your electronic blood glucose self-monitoring records for the preceding 3 months

Chronic Kidney Disease

(i.e.: CKD Stages 1-4)

– Bring a letter from your Nephrologist (kidney specialist) that outlines your medical history, current medications, current state of your health, and that indicates it is safe for you to drive a CMV

– Bring recent (within the past 3 months) EKG 

– Note: Stage 4 CKD must bring EKG and Echocardiogram 

– Individuals who require renal replacement therapy (dialysis) and those with stage 5 CKD who are not receiving renal replacement therapy cannot be considered as fit-for-duty 

Neurological

(i.e.: stroke, brain tumor, seizure disorder, brain hemorrhage/bleed, aneurysm)

– Bring a letter from your Neurologist (brain or nerve specialist) that outlines your medical history, current medications, current neurologic and psychiatric state, and that indicates it is safe for you to drive a CMV

Missing Limb

(i.e.: hand, finger, arm, foot, leg)

– Bring an overview from your specialists (Orthopedic, Physiatry) of the injury and if you have any work restrictions due to the injury

– Please provide a Skilled Performance Examination to qualify for your DOT card

High-Risk Medications​

(i.e.: sedatives, controlled substances, narcotics, sleeping pills, anxiety and ADHD medications)

Download the CMV Driver Medication Form

– Have your prescribing provider fill the form out and bring it with you to your DOT exam

– You may also need a letter regarding the safety of driving a Commercial Motor Vehicle while using these medications

 Coumadin:

– Bring a recent (within the past 30 days) INR blood level and a letter of clearance from your treating physician

– INR level check must be done every 30 days

Forms and Additional Information 

ALL DRIVERS 

 

Have your provider or specialist complete the Safety Medical Release Opinion Form and any of the above required forms specific to your health condition.

 

Print the Medical Examination Report Form and bring the completed form with you to your DOT physical appointment. If you cannot print and complete the form prior to your appointment, no worries, we have them on-site.

Arrive 15-20 minutes early for your DOT appointment to complete the form in the office.
